All health conditions should be evaluated in detail before tooth extraction
Underlining that patients with various diseases, especially heart disease, should be careful in tooth extraction, Prof. Dr. Rüştü Cem Tanyel said that if the patient has any systemic disease, it should be carefully handled and evaluated accordingly. Tanyel said, “Extraction is risky in any situation that will prevent any operation from being performed in the body. Patients with uncontrolled hypertension, patients who have recently had a heart attack and patients with heart failure are in the high risk group.”
Prof. Dr. Rüştü Cem Tanyel, Professor of Oral, Dental and Maxillofacial Surgery at Istanbul Atlas University Faculty of Dentistry, pointed out the points to be considered in tooth extraction and gave important warnings.
The patient should be well evaluated before tooth extraction
Stating that tooth extraction is a more complicated procedure than many of the other treatments performed in different areas of the body since it involves both bone and soft tissue, Prof. Dr. Rüştü Cem Tanyel said that when a tooth is decided to be extracted, the patient's entire health status should be evaluated in detail and carefully questioned before the extraction.
Caution should be taken in heart patients

Stating that patients with bleeding disorders, severe anemia and hemophilia should also be carefully evaluated, Prof. Dr. Rüştü Cem Tanyel said, “Necessary precautions should be taken in patients with liver and kidney failure. Caution should be exercised in patients with any disease that reduces body resistance or in patients taking any medication that will reduce body resistance. This includes patients with uncontrolled diabetes, autoimmune disorders and transplant patients. Chemotherapy patients undergoing cancer treatment are also in the risk group, considering that their body resistance is weakened.”
Patients with allergies should first consult their doctor
Stating that patients who are known to be allergic to various foods and medications should also be carefully examined, Prof. Dr. Rüştü Cem Tanyel said, “Patients with such diseases should be referred to their own doctors before the extraction, re-examined and find out if there are any contraindications for the extraction. The physician who will perform the extraction makes a consultation with the patient's doctors, the doses of the drugs used are adjusted and the extraction is performed. If the patient is aware of these complaints but does not receive any treatment, they should be referred to the necessary physicians and systemic problems should be treated first.”
The safest period for pregnant women: Second trimester
Prof. Dr. Rüştü Cem Tanyel stated that the second trimester is the safest time for extraction in pregnant patients and underlined that the approval of the patient's doctor should be obtained in other periods.
What should be considered in tooth extraction?
Underlining that appropriate conditions must be provided for tooth extraction, Prof. Dr. Rüştü Cem Tanyel said, “In patients who do not have a systemic problem or in patients who have a systemic problem and are under control, the extraction procedure should be performed by competent people using appropriate tools under sterile office or clinic conditions.”
Recommendations of the physician should be followed after the extraction
Prof. Dr. Rüştü Cem Tanyel said that the recommendations given by the physician should be strictly followed after tooth extraction, “If any medication is prescribed, it should be used regularly. Since extraction is a minor surgical procedure, swelling and pain lasting one week to ten days is normal. In these cases, antibiotics or painkillers may be required. In patients with undiagnosed bleeding disorders, bleeding may not stop. Patients with poor oral hygiene may develop an infection at the extraction site. Alveolitis may occur due to the inability of the clot in the extraction cavity to organize. This may cause pain even worse than toothache,” he warned.
Timely treatment of caries prevents inflammation
Prof. Dr. Rüştü Cem Tanyel, who also touched upon what to do especially in cases of inflammation that occur before extraction and inflammation that occurs afterwards, said, “Inflammation does not occur in a healthy tooth. Oral and dental care should be taken to prevent inflammation. Timely treatment of caries prevents progression and infection. If an infection has occurred in a tooth, the physician decides on the extraction and time. If the infection is an acute infection, the doctor may ask for antibiotics to be used before extraction. However, chronic infections do not require antibiotics before extraction. The first treatment of any infection in the body is to eliminate the effect. If the causative agent is a tooth and cannot be treated, it would be appropriate to remove it from the area immediately.”
Pay attention to these points before shooting
Prof. Dr. Rüştü Cem Tanyel, who also drew attention to what should be done before extraction in diseases such as various heart diseases and febrile rheumatism, concluded his words as follows:
“There is a popular belief that an inflamed tooth cannot be extracted. In a healthy individual, whether the infection is acute or chronic, extraction can be performed if the area can be numbed with local anesthesia. In such procedures, bacteria from the infected area may be mixed into the blood, but this is not a problem in a healthy individual with good body resistance and a properly functioning defense system. These bacteria can cause life-threatening systemic problems in patients with low body resistance or in patients with problems in the heart tissues due to previous febrile rheumatism. Therefore, patients with febrile rheumatism, patients with untreated heart valve problems or defects, or patients with artificial heart valves undergo special procedures before extraction.”
